Job summary

Home Sweet Home Health Agency is seeking a compassionate Occupational Therapist (OT) to join our Home Health team in East Valley, Arizona. This Per Diem (PRN) role offers flexible scheduling and the chance to provide one-on-one in-home care to patients in Scottsdale, Tempe, Chandler, and Gilbert.

As an OT with Home Sweet Home, you will evaluate patients at home, create individualized ADL-focused plans, and collaborate with an interdisciplinary team to support recovery and independence.

Qualifications

• Licensed OT in Arizona (OT license) is required.
• Graduate of an ACOTE-accredited Occupational Therapy program.
• Minimum one year of OT experience; Home Health preferred.

Responsibilities

• Conduct in-home OT evaluations and reassessments.
• Develop individualized treatment plans to improve ADLs independence.
• Provide rehab for patients recovering from illness, injury, surgery, or hospitalization.
• Recommend adaptive equipment, home modifications, and fall prevention strategies.
• Educate patients and caregivers on safety, mobility, and functional independence.
• Collaborate with physicians, nurses, PTs, SLPs, and the interdisciplinary care team.
• Document progress in EMR while ensuring Medicare and agency compliance.

Occupational Therapist Responsibilities

As a Home Health Occupational Therapist , you will:

• Complete in-home Occupational Therapy evaluations and reassessments
• Develop individualized treatment plans to improve independence with Activities of Daily Living (ADLs)
• Provide rehabilitation for patients recovering from illness, injury, surgery, or hospitalization
• Recommend adaptive equipment, home modifications, and fall prevention strategies
• Educate patients and caregivers on safety, mobility, and functional independence
• Collaborate with physicians, nurses, Physical Therapists (PT), Speech-Language Pathologists (SLP), and the interdisciplinary care team
• Document patient progress accurately using EMR while maintaining Medicare and agency compliance

Qualifications

• Current Arizona Occupational Therapist (OT) license
• Graduate of an ACOTE-accredited Occupational Therapy program
• Minimum one year of Occupational Therapy experience (Home Health preferred)
• Strong communication, organization, and time management skills
• Experience with EMR documentation
• Valid driver’s license, reliable transportation, and current auto insurance
