Job summary

Home Sweet Home Health is seeking a compassionate Occupational Therapist (OT) to join our Home Health team serving the East Valley, including Scottsdale, Tempe, Chandler and Gilbert. This per diem (PRN) role offers meaningful, one-on-one in-home therapy to help patients regain independence.

You will evaluate and treat ADLs, develop individualized plans, recommend adaptive equipment, educate patients and caregivers, and collaborate with an interdisciplinary team while documenting progress in EMR

Qualifications

• Current Arizona OT license.
• Graduate of an ACOTE-accredited OT program.
• Minimum one year OT experience; Home Health preferred.
• Strong communication, organization and time management skills.
• Experience with EMR documentation.
• Valid driver's license, reliable transportation and current auto insurance.

Responsibilities

• Complete in-home OT evaluations and reassessments.
• Develop individualized treatment plans to improve ADLs.
• Provide rehabilitation for patients recovering from illness, injury, surgery or hospitalization.
• Recommend adaptive equipment, home modifications and fall prevention strategies.
• Educate patients and caregivers on safety, mobility and functional independence.
• Collaborate with physicians, nurses, PTs, SLPs and the interdisciplinary care team.
• Document patient progress in EMR and ensure Medicare/agency compliance.
