Talin Home Healthcare seeks a dedicated Occupational Therapist to deliver high-quality care in patients' homes across Fairfax area. You will assess, plan, and implement therapies to improve daily living skills and functionality, collaborating with speech therapists and case managers.

Required OT degree and state licensure; strong communication and compassionate patient focus are essential for success in this role.

Qualifications

• Degree in Occupational Therapy from an accredited program.
• Valid state licensure as an Occupational Therapist.
• Strong knowledge of physiology and occupational health principles.
• Experience in case management with individuals with disabilities preferred.
• Familiarity with early intervention strategies is a plus.
• Excellent communication with patients, families and interdisciplinary teams.
• Ability to develop effective care plans tailored to patients.
• Compassionate, patient-focused approach to rehabilitation.

Responsibilities

• Conduct comprehensive evaluations to assess patients' physical, emotional, and social needs.
• Develop and implement individualized care plans tailored to each patient's goals and capabilities.
• Provide direct patient care through therapeutic activities to improve daily living skills.
• Collaborate with other healthcare professionals to ensure holistic patient care.
• Educate patients and families about treatment options, progress, and strategies.
• Monitor progress and adjust treatment plans as needed based on outcomes.
• Maintain accurate documentation of assessments and progress notes.
