Job summary

Cheshire West and Chester Council is recruiting an Occupational Therapist to join the Home First Intermediate Care Team. This full-time, permanent role operates across the Cheshire West footprint with hybrid flexible working.

You will assess people post-discharge from hospital, visit homes to review moving and handling, advocate for proportionate care, and document assessments in Liquid Logic, collaborating with health colleagues to achieve therapy goals.

Qualifications

• Qualified occupational therapist registered with the HCPC.
• Experience in moving and handling assessment and practice.
• Experience in social care/hospital discharge contexts.

Responsibilities

• Support social workers with post-discharge assessments.
• Visit patients at home to review moving and handling.
• Advise and challenge to promote proportionate care.
• Complete evidence in Liquid Logic and risk assessments.
• Collaborate with health colleagues in hubs and attend huddle meetings.
