Bear Mountain is seeking a compassionate Occupational Therapist to join our rehabilitation team. You will evaluate residents and develop individualized plans to improve function, safety, and independence, collaborating with an interdisciplinary team to achieve meaningful outcomes.

You will provide patient-centered therapy, support residents through recovery, and contribute to discharge planning while maintaining compliant documentation and ongoing professional development.

Qualifikationen

• Graduate of an accredited Occupational Therapy program.
• Current state license to practice Occupational Therapy.
• Long-term care, skilled nursing, or rehabilitation experience preferred.
• Strong clinical reasoning and treatment planning skills.
• Excellent communication and interpersonal skills.
• Ability to collaborate effectively with residents, families, and interdisciplinary teams.
• English language proficiency required.

Aufgaben

• Evaluate, develop, and implement individualized occupational therapy programs to improve function, safety, and independence.
• Perform occupational therapy evaluations and assessments based on resident needs.
• Develop treatment plans focusing on daily living skills, mobility, cognition, safety, and independence.
• Provide skilled OT treatments per physician orders and plans of care.
• Educate residents and families on strategies to promote independence and safety.
• Participate in discharge planning and interdisciplinary team meetings.
• Provide clinical direction to OT assistants and maintain documentation.
