Qualifications

Licensed Occupational Therapist
Open to applicants of all experience levels

Responsibilities

Screen and evaluate patients with cognitive and/or ADL functional disorders and develop plan of care.
Utilize standardized assessments and evidence-based practice to support interventions.
Provide rehabilitative, skilled and medically necessary treatment per plan and regulatory standards.
Administer ADL retraining, therapeutic activities, exercises, modalities, and use of assistive devices.
Assess progress and adjust treatments in the clinical record and refer as needed.
Evaluate outcomes and modify services based on patient progress and referrals.
Provide counseling to patients and families and coordinate with the health team.
Prepare comprehensive discharge summaries per regulatory requirements.
