Job summary

The Grand Rehabilitation and Nursing at Rome in Rome, NY is seeking a dedicated Occupational Therapist to join our care team. You will evaluate residents, develop and implement treatment plans, track progress, and document notes to support residents' functional independence and well-being.

Requirements include graduating from an accredited Occupational Therapy Program and current New York State OT license; experience is a plus.

Responsibilities

• Evaluate and develop treatment plans for residents based on medical diagnoses and goals.
• Implement planned treatments to improve residents' functional independence.
• Assess and document progress in recertifications, progress reports, and daily notes.
• Maintain resident confidentiality and safeguard confidential information.
• Audit records to ensure documentation accuracy and compliance.
• Supervise OT assistants and participate in care plan meetings and documentation.
• Prepare OT-related reports, data tracking, and projects as directed.
