Job summary

TriHealth is offering a grant-funded Occupational Therapist opportunity in our Community Benefits program at Bethesda North Hospital in Montgomery, OH. This role provides a unique chance to expand your expertise in a supportive, collaborative environment, with an optional schedule.

You will evaluate patients, plan and provide interventions in a transdisciplinary team, supervise staff, and contribute to program development and education.

Qualifications

  • Bachelor's degree required.
  • BLS certification required.
  • State licensure required.
  • Up to 1 year experience in a related field.

Responsibilities

  • Connecting with and monitoring fall prevention process in physician practices via Epic
  • Education and training with Physician practice staff
  • Triaging high risk fall referrals from physicians
  • Providing follow up phone calls to high risk fall patients
  • Providing connection to community education programs as indicated
  • Completing Occupational Therapy fall prevention home visits for grant funded program as indicated.
  • Gathering fall prevention data from Epic for grant reports and internal purposes as indicated.
  • Deliver Evidence Based Fall prevention programs (either Stepping On or Tai Chi for Health), once training and certification is completed.

Job Overview

Evaluates patients, plans for and provides therapeutic interventions in a transdisciplinary/team approach. Responsible for the communication and coordination of treatment programs with other team members including the patient/family. Directly supervises assistants, interns and other support personnel, participates in program developments, continuous improvement and clinical education; and provides in-services to staff, hospital personnel and the community. Will also be expected to be a member of a collaborative team which is patient focused, functional outcome based and patient/family centered.
