Job summary

GEM Transition Care Center in Pasadena, CA is seeking a licensed Occupational Therapist to evaluate and treat residents with physical, cognitive, and psychosocial impairments to maximize independence and quality of life.

You will develop individualized treatment plans, provide skilled OT interventions, educate residents and families, document progress, and collaborate with an interdisciplinary team while keeping up-to-date with OT standards and regulations.

Qualifications

• Current license to practice as an Occupational Therapist in California.
• Minimum 2 years of healthcare experience, preferably in a skilled nursing facility.
• Proficient in using electronic medical records and other healthcare software.
• Knowledge of OT practice standards and regulations.

Responsibilities

• Evaluate residents' physical, cognitive, and psychosocial abilities using standardized assessments.
• Develop and implement individualized treatment plans based on assessments and physician orders.
• Provide OT interventions including therapeutic exercise, ADL training, cognitive retraining, and environmental modifications.
• Monitor resident progress and adjust treatment plans as needed.
• Coordinate care with physicians, nurses, PTs, and SLPs to optimize outcomes.
• Educate residents and families on goals, plans, and home exercise programs.
• Maintain accurate documentation of OT services.
• Participate in team meetings, in-service education, and quality improvement initiatives.
• Maintain current knowledge of OT practice and regulations.
