Job summary

HealthMark Rehabilitation is seeking an Occupational Therapist/OTR for Willowbrook Nursing Center in Nacogdoches, TX. The role supports residents and short-term patients with skilled therapy services in a hospital-like rehabilitation setting.

Full-time or part-time positions are available. The therapist will develop and implement individualized care plans, document progress, and collaborate with an interdisciplinary team to support discharge planning and optimal patient outcomes.

Qualifications

• Must hold Texas license/certification for Occupational Therapy (OTR).
• Strong oral and written communication skills required.
• Ability to function in multi-faceted systems and interact with all staff levels.

Responsibilities

• Provides therapy services per physician orders and standards of practice.
• Develops and follows a plan of care with short and long-term goals for discharge.
• Documents patient progress and responses to interventions in a timely manner.
• Completes all required documentation and billing per state/federal regulations.
• Maintains rehab gym equipment and treatment area with infection prevention in mind.
