Job summary

Renewal Rehab, part of Enhance Therapies, is hiring a licensed Occupational Therapist for a full-time role in a skilled nursing facility in Coleta, IL. The position requires a current OT license and NBCOT certification where required by state law. Compensation is hourly and benefits are included.

You will evaluate patients, develop care plans, collaborate with a multidisciplinary team, and maintain regulatory documentation while supporting patient independence and quality of life.

Qualifications

• Must be a licensed Occupational Therapist to apply.
• Current license as an Occupational Therapist in the state of practice.
• NBCOT certification may be required by state.

Responsibilities

• Evaluate and treat patients with functional, cognitive, and ADL impairments.
• Create and implement personalized care plans.
• Collaborate with team members to ensure quality outcomes.
• Maintain documentation to meet all regulatory requirements.
• Ensure all physician orders are obtained and up to date.
