Continuum Therapy Partners seeks an Occupational Therapist to join our therapist-led team at Avalon Health and Rehab in Mount Vernon, OH. The position features a daytime, full-time schedule with comprehensive benefits and licensure support.

The ideal candidate holds a Master’s or Doctorate in OT, current state license with NBCOT certification, and prefers 1 year of OT experience. You will evaluate clients, develop treatment plans, document progress, and educate families on equipment and safety.

Qualifications

• Master's or Doctorate in Occupational Therapy required
• Current, unencumbered state Occupational Therapy License; NBCOT Certification required
• 1 year of previous experience as an Occupational Therapist preferred
• Evaluates and develops individualized treatment plans to improve patients' functional independence
• Documents patient progress and modifies treatment plans based on clinical outcomes
• Educates patients and families on adaptive equipment and home safety recommendations
