Flagship Therapy in Panorama City, CA seeks a full-time Occupational Therapist to join our in-house rehab program. You will screen and treat clients with cognitive and ADL impairments, develop care plans, and collaborate with an in-house therapy team to promote recovery.

Qualifications

• Licensed OT with state licensure.

Responsibilities

• Screen/evaluate patients with cognitive and/or ADL impairments and develop care plans.
• Utilize standardized assessments to guide interventions.
• Provide rehabilitative, skilled treatment per plan and regulatory standards.
• Administer procedures as part of the rehabilitation plan (ADL retraining, activities, exercises, modalities).
• Assess progress and adjust treatments in the clinical record.
• Evaluate outcomes and refer based on results.
• Provide counseling to patients and families.
• Communicate progress and plans with supervisor and health team.
• Provide comprehensive discharge summaries per regulatory requirements.
