Pacific Care and Rehabilitation in Hoquiam, WA is a post-acute care facility offering long-term, short-term and geriatric services. We are seeking a full-time Occupational Therapist for our in-house rehab program to join a stable and fun therapy team.

As an OT, you will screen and treat patients with cognitive and ADL disorders, use standardized assessments, document progress, and contribute to discharge planning while collaborating with the health team.

Qualifications

• Licensed Occupational Therapist.
• Open to applicants of all experience levels.

Responsibilities

• Screen/evaluate patients with cognitive and/or ADL disorders and develop plan of care.
• Use standardized assessments and evidence-based practice to guide interventions.
• Provide rehabilitative, skilled and medically necessary treatment per clinical requirements.
• Administer procedures as part of the rehabilitation plan (ADL retraining, therapeutic activities, exercises, modalities, assistive devices).
• Assess progress and adjust treatments in the clinical record.
