Job summary

Putnam Nursing & Rehabilitation Center in New York is seeking a licensed Occupational Therapist (OT) to provide PRN rehabilitation services within our 160-bed SNF. The OT will evaluate and treat geriatric patients with complex medical and functional needs, developing individualized plans to improve mobility, ADLs, and independence.

The role supports both full-time and per diem schedules, with interdisciplinary collaboration and an emphasis on patient-centered care.

Qualifications

• Must possess current license and registration to practice as an Occupational Therapist (OT) in NY.
• Must possess current malpractice insurance, license in good standing.
• Energetic, enthusiastic, and passionate about quality care.
• Excellent communication, customer service, and organization skills.

Responsibilities

• Licensed OT providing PRN (as-needed) rehabilitation services in the facility.
• Evaluating and treating geriatric patients with complex medical, cognitive, and functional needs.
• Developing individualized treatment plans to improve mobility, daily living skills, and independence.
• Collaborating with interdisciplinary teams including nurses, physicians, speech therapists, and physical therapists.
• Skilled in adaptive equipment training, fall prevention, and patient/caregiver education.
• Flexible and responsive, delivering high-quality care across variable schedules and patient acuity levels.
