Job summary

Benedictine is seeking an Occupational Therapist to support residents with comprehensive evaluation, ongoing assessment, and planning of therapy interventions. The role develops and revises care plans with functional goals and discharge planning, and may involve travel to multiple BHS sites to deliver treatment.

The therapist will educate families and care teams on the care plan and participate in discharge coordination as residents’ needs evolve.

Qualifications

• Must hold current state licensure as an Occupational Therapist in good standing.
• Bachelor’s degree in Occupational Therapy required.

Responsibilities

• Evaluate residents and conduct ongoing assessments.
• Plan and implement therapy interventions and revise plan of care as needed.
• Develop functional goals, outcomes, and discharge plans.
• Educate residents, family, and caregivers about the care plan; assist with discharge planning.
• Provide treatment across multiple BHS communities and travel between sites as required.

Job description

The Occupational Therapist Float is responsible and accountable for residents’ evaluation, ongoing assessment, planning, and implementation of therapy interventions.

Develops and revises plan of care based on the residents’ needs and establishes functional goals, outcomes, and discharge plan. Plans for residents’ discharge and anticipates for long term and/or short term residents’ needs. Educates other health care team members, residents, family, and caregivers on information regarding the care plan for the residents. Provides treatment to all BHS communities as needed; may be required to travel to multiple sites in a day.

Qualifications Required

• Current state licensure as an Occupational Therapist, in good standing.
• Bachelor’s degree.
