Job summary

LiveWell Homecare Agency, LLC is seeking an Occupational Therapist to assess, plan, and implement rehabilitation programs improving daily activities, mobility, cognition, strength, and ROM for patients across care settings.

You will collaborate with physicians, educate families, document progress, and uphold ethical standards while supporting a collaborative care team and ongoing staff onboarding.

Job description

Role: Occupational Therapist responsible for assessing, planning, and implementing rehabilitative programs to improve patients’ daily activities, mobility, cognition, strength, coordination, and range of motion. Services are delivered across the continuum, emphasizing quality, cost-effectiveness, and regulatory compliance.

Key Responsibilities:

• Patient Assessment: Collaborate with physicians to establish and evaluate treatment plans, goals, and discharge strategies based on patient needs.
• Treatment Planning & Care: Implement therapy plans, select appropriate activities, revise goals with patient input, and provide education, discharge instructions, and referrals.
• Documentation: Record evaluations, treatment sessions, family education, and discharge details per departmental protocols.
• Professional Conduct: Uphold ethical standards, maintain confidentiality, punctuality, and effective communication. Participate in meetings, training, and process improvements.
• Customer Service & Teamwork: Deliver compassionate care, apply AIDET principles, foster positive interactions, and support a collaborative environment.
• Professional Development: Engage in ongoing education, complete required competencies, and assist in onboarding new staff.

Qualifications:

• Degree from an accredited occupational therapy program (Bachelor’s or Master’s in Occupational Therapy).
• Licenses and certifications including Occupational Therapist credential, BLS, valid Georgia driver’s license, and vehicle insurance.
• Effective communication, customer service orientation, team player, and basic computer skills.
