This role focuses on geriatric and post-acute care, with collaboration across an interdisciplinary team to improve mobility, balance, and independence. California license requirements apply, and a supportive team environment is offered.

Qualifications

• Graduate of an accredited Occupational Therapy program.
• Current or eligible California Occupational Therapist license.
• Strong communication and documentation skills.
• Ability to work effectively as part of an interdisciplinary rehabilitation team.
• SNF or post-acute experience is preferred, but qualified candidates from other settings are encouraged to apply.

Responsibilities

• Evaluate patients and develop individualized treatment plans.
• Provide skilled physical therapy services to help patients improve mobility, strength, balance, and functional independence.
• Establish measurable short- and long-term therapy goals.
• Complete accurate and timely clinical documentation.
• Collaborate with OT, SLP, nursing, physicians, and other members of the interdisciplinary team.
• Educate patients, families, and caregivers regarding treatment and discharge recommendations.
• Participate in care planning and discharge planning as appropriate.
