Job summary

Qualifications

• Graduate of an accredited Occupational Therapy program.
• Current or eligible California Occupational Therapist license.
• Strong communication and documentation skills.
• Ability to work effectively as part of an interdisciplinary rehabilitation team.
• SNF or post-acute experience is preferred, but qualified candidates from other settings are encouraged to apply.

Responsibilities

• Evaluate patients and develop individualized treatment plans.
• Provide skilled physical therapy services to help patients improve mobility, strength, balance, and functional independence.
• Establish measurable short- and long-term therapy goals.
• Complete accurate and timely clinical documentation.
• Collaborate with OT, SLP, nursing, physicians, and other members of the interdisciplinary team.
• Educate patients, families, and caregivers regarding treatment and discharge recommendations.
• Participate in care planning and discharge planning as appropriate.

Setting: Skilled Nursing Facility (SNF)

We are seeking a compassionate and motivated Occupational Therapist (PT) to join a skilled nursing rehabilitation team in Sacramento, California. We are flexible and can consider candidates interested in Full-Time, Part-Time, or PRN schedules.

This is an excellent opportunity for a Physical Therapist who enjoys working with the geriatric and post-acute population and wants flexibility while being part of a supportive rehabilitation team.
