Job summary

Element Health Services is seeking a licensed Occupational Therapist to deliver therapy services in Florida, including evaluations, care planning, and home/community-based care for clients in Independent and Assisted Living Facilities.

You will collaborate with a multidisciplinary team, fabricate splints, train on devices, and participate in discharge planning to support patient independence and safety.

Qualifications

• Graduate of an accredited Occupational Therapy program recognized by the AMA and AOTA.
• Registered to practice OT (NBCOT).
• Licensed to practice as an OT in Florida.
• Minimum of two years of clinical OT experience.
• Current BLS Certification.
• Strong verbal and written communication and organizational skills.

Responsibilities

• Assess the patient's functional status (muscle function, endurance, visual coordination, communication skills, self-care, work capacity, etc.).
• Collaborate in developing a comprehensive care plan.
• Design a treatment program with goals for improved function and communicate the plan to the referring physician and other team members.
• Teach or retrain patients in daily activities lost due to illness or injury, following company policies.
• Fabricate splints and instruct patients on using adaptive equipment to enhance function.
• Provide training in the use of prosthetic and/or orthotic devices.
• Maintain accurate clinical records and report any changes in the patient's condition to the physician.
• Identify patient and family/caregiver needs for additional services and make appropriate referrals.
• Educate patients' families/caregivers and other team members on the patient's treatment plan.
• Supervise certified occupational therapy assistants in accordance with organization policies and state regulations.
• Participate in discharge planning.
