Job summary

Element Health Services is seeking a licensed Occupational Therapist to deliver therapy in community and home settings for patients in Independent and Assisted Living Facilities.

You will evaluate patients, create care plans, and guide treatment to improve daily functioning. Strong communication and organizational skills are essential, with supervision of assistants and adherence to Florida OT laws.

Qualifications

• Graduate of an accredited Occupational Therapy program recognized by the AMA and AOTA.
• Licensed to practice as an OT in Florida and registered to sit for the NBCOT exam.
• Minimum of two years of clinical experience as an OT.
• Current BLS Certification.
• Strong verbal and written communication and organizational skills.

Responsibilities

• Assess functional status and plan care.
• Collaborate to develop comprehensive care plans.
• Teach daily activities and retraining as needed.
• Fabricate splints and instruct use of adaptive devices.
• Provide training in prosthetic/orthotic device use.
• Maintain accurate clinical records and communicate changes to physicians.
• Identify patient/family needs for additional services and refer as appropriate.
• Educate families and team members about treatment plans.
• Supervise OT assistants per policy and state regulations.
• Participate in discharge planning.
