Evolve Therapy Services is seeking an Occupational Therapist for an in-house Skilled Nursing Facility in Rome, NY. This full-time or PRN role involves planning, directing OT services in a facility to ensure high-quality resident care in compliance with applicable standards and regulations.

You will screen residents, develop treatment plans, communicate findings, coordinate schedules with the care team, document and bill for services, and supervise COTA as needed to uphold clinical excellence.

Competenze

• Active Occupational Therapy license in the practicing state(s).
• Ongoing continuing education to maintain licensure and certification.
• Degree in Occupational Therapy.

• Conduct regular resident screenings to determine appropriate OT interventions.
• Evaluate residents to inform and develop treatment plans with short- and long-term goals.
• Interpret and communicate evaluation findings and treatment plans to residents, families, and interdisciplinary team.
• Establish individual resident therapy schedules with input from the team.
• Perform therapy according to treatment plans and ensure proper documentation and billing.
• Oversee COTA as needed and ensure treatments are carried out with clinical excellence.

Job Position: Occupational Therapist for an In-house Skilled Nursing Facility

Job Type: Full Time or PRN positions available

Job Purpose: To plan, organize, develop, and direct Occupational Therapy Services in facility in accordance with current applicable federal, state, and local standards, guidelines, and regulations, and as may be directed by Administrator, to ensure that highest degree of quality resident care can be maintained at all times.

Job Functions:

• Conduct regular screening of residents to figure out the best intervention or treatment for each individual patient
• Evaluate residents to inform and develop treatment plans; establish short- and long-term goals and methods to achieve goals
• Interpret and communicate evaluation findings, treatment plans, and recommendations to residents, families/caregivers, and interdisciplinary team members
• Establish individual resident therapy schedules in cooperation with other team members
• Conduct therapy according to established treatment plan and ensure proper documentation and billing of occupational therapy services
• Perform all other duties as assigned and as required to effectively treat patients
• Oversee COTA as needed and ensure all documentation and treatments are carried out with clinical excellence and proper reimbursement practices are being followed

Education & Qualifications:

• Degree in Occupational Therapy
• MUST have an Active Occupational Therapy license in practicing state(s)
• Ongoing continuing education and professional development to maintain current licensure and certification
