Job summary

Evolve Therapy Services is seeking an Occupational Therapist to join our in-house Skilled Nursing Facility partner in Massena, NY. This position offers full-time, part-time, or PRN opportunities, with flexible schedules to fit your availability.

You will plan, develop, and direct OT services, screen residents, create treatment plans with goals, and collaborate with a multidisciplinary team. Active OT license in practicing state required; continuing education encouraged.

Qualifications

• Active OT license in practicing state(s) is required.
• Degree in Occupational Therapy is required.
• Ongoing continuing education to maintain licensure is expected.

Responsibilities

• Screen residents to determine best interventions for each patient.
• Evaluate residents and develop treatment plans with short- and long-term goals.
• Communicate evaluation findings, plans, and recommendations to residents, families, and the care team.
• Establish individual resident therapy schedules with the team.
• Deliver therapy per plan and ensure proper documentation and billing.
• Perform other duties as assigned to effectively treat patients.
• Oversee COTA as needed and ensure documentation and reimbursement are compliant.

Job Position: Occupational Therapist for an In-house Skilled Nursing Facility

Job Type: Full Time, Part Time, or PRN positions available

Job Purpose: To plan, organize, develop, and direct Occupational Therapy Services in facility in accordance with current applicable federal, state, and local standards, guidelines, and regulations, and as may be directed by Administrator, to ensure that highest degree of quality resident care can be maintained at all times.

Job Functions:

• Conduct regular screening of residents to figure out the best intervention or treatment for each individual patient

• Evaluate residents to inform and develop treatment plans; establish short- and long-term goals and methods to achieve goals

• Interpret and communicate evaluation findings, treatment plans, and recommendations to residents, families/caregivers, and interdisciplinary team members

• Establish individual resident therapy schedules in cooperation with other team members

• Conduct therapy according to established treatment plan and ensure proper documentation and billing of occupational therapy services

• Perform all other duties as assigned and as required to effectively treat patients

• Oversee COTA as needed and ensure all documentation and treatments are carried out with clinical excellence and proper reimbursement practices are being followed

Education & Qualifications:

• Degree in Occupational Therapy

• MUST have an Active Occupational Therapy license in practicing state(s)

• Ongoing continuing education and professional development to maintain current licensure and certification
