Evolve Therapy Services is seeking an Occupational Therapist for an in-house Skilled Nursing Facility partner located in Tupper Lake, NY. The role offers Full Time, Part Time or PRN positions with a sign-on bonus and competitive benefits.

You will plan, organize, develop, and direct occupational therapy services per federal and state standards, ensuring high-quality resident care and proper documentation and billing. Collaboration with an interdisciplinary team is essential.

Qualifications

• Must have an Active Occupational Therapy license in practicing state(s).
• Degree in Occupational Therapy from an accredited program.
• Continued education to maintain licensure.

Responsabilités

• Screen residents to determine best intervention or treatment for each patient.
• Evaluate residents and develop treatment plans with short- and long-term goals.
• Interpret and communicate evaluation findings to residents, families, and the care team.
• Establish resident therapy schedules with other team members.
• Conduct therapy per plan and document and bill services.
• Oversee COTA and ensure documentation and treatments are carried out with good reimbursement practices.
• Perform all other duties as assigned to effectively treat patients.

Job Position: Occupational Therapist for an In-house Skilled Nursing Facility

Job Type: Full Time (Sign On Bonus!), Part Time, or PRN positions available

Job Purpose: To plan, organize, develop, and direct Occupational Therapy Services in facility in accordance with current applicable federal, state, and local standards, guidelines, and regulations, and as may be directed by Administrator, to ensure that highest degree of quality resident care can be maintained at all times.

Job Functions:

• Conduct regular screening of residents to figure out the best intervention or treatment for each individual patient




• Evaluate residents to inform and develop treatment plans; establish short- and long-term goals and methods to achieve goals




• Interpret and communicate evaluation findings, treatment plans, and recommendations to residents, families/caregivers, and interdisciplinary team members




• Establish individual resident therapy schedules in cooperation with other team members




• Conduct therapy according to established treatment plan and ensure proper documentation and billing of occupational therapy services




• Perform all other duties as assigned and as required to effectively treat patients




• Oversee COTA as needed and ensure all documentation and treatments are carried out with clinical excellence and proper reimbursement practices are being followed

Education & Qualifications:

• Degree in Occupational Therapy




• MUST have an Active Occupational Therapy license in practicing state(s)




• Ongoing continuing education and professional development to maintain current licensure and certification
