Job summary

Warsaw Rehabilitation and Healthcare Center in Warsaw, NC is hiring an Occupational Therapist to join a multidisciplinary team providing quality resident care. The role includes evaluations, treatment planning, documentation, and discharge planning to support patient progress.

The position requires an OT degree recognized by the AOTA and NC licensure, with SNF experience preferred. A flexible schedule and comprehensive benefits are offered to support staff well-being and professional growth.

Qualifications

• Graduate of an accredited university with a B.S., M.A., M.S. or Doctorate in Occupational Therapy recognized by the AOTA.
• Holds current license/registration in the state of NC (as applicable).
• SNF experience preferred.

Responsibilities

• Provide a comprehensive occupational therapy evaluation based on MD orders.
• Document findings on the standardized evaluation form timely and accurately.
• Provide a comprehensive treatment plan with long and short-term goals, frequency, duration, and modalities.
• Ensure MD orders are obtained for evaluations, treatments, recertifications and discharges.
• Deliver comprehensive treatment using modalities and adjust plans as indicated while complying with precautions.
• Document patient daily participation and progress accurately.
• Update treatment plans monthly for recertification, comprehensively and timely.
• Document discharge summaries when indicated.
• Complete caregiver or discharge in-services as required.
• Complete Facility documentation forms and chart care plans for the MDS as needed.
• Attend facility and rehab meetings as directed by Supervisor/Regional Manager.
• Participate in facility committees and act as rehab delegate to the interdisciplinary team.
• Assist in scheduling patients and performing screens as indicated.
• Complete billing and bill patients ethically for OT services rendered.
• Cover other therapists’ caseload during absences as necessary.
• Supervise junior staff, students and technicians as needed; co-sign notes as indicated.
• Maintain professional relations with staff, patients and families.
• Provide in-service education to rehab staff and facility/hospital staff as needed.
• Comply with all facility policies, confidentiality and Resident Rights.
• Be punctual, adhere to hours and project a professional image at all times.
• Adhere to productivity requirements and HR policies regarding lunch, sick and time off.
• Maintain a positive work atmosphere and foster good relations with customers and colleagues.
• Perform any other duties as assigned by supervisor or Regional Manager.
