Job summary

Fletcher Rehabilitation and Healthcare Center in Fletcher, NC is seeking an Occupational Therapist to join our long-term care team. You will evaluate residents, develop treatment plans, and coordinate care with the interdisciplinary team to improve functional outcomes.

The OT will document progress, ensure MD orders are followed, attend meetings, and participate in facility initiatives. A valid state license and AOTA recognition are required, with SNF experience preferred.

Qualifications

• Graduates with a B.S., M.A., M.S. or Doctorate in Occupational Therapy recognized by AOTA.
• SNF experience preferred.
• Holds current license/registration in the state where practice occurs.

Responsibilities

• Provide comprehensive OT evaluation based on MD orders.
• Document findings on standardized evaluation forms timely and accurately.
• Develop comprehensive treatment plans with short and long-term goals, frequency and duration.
• Obtain MD orders for evaluations, treatments, recertifications and discharges.
• Provide treatments using modalities while adhering to precautions and updating progress notes.
• Document daily participation and progress in progress notes.
• Update treatment plans at recertification and document discharge summaries as indicated.
• Document caregiver or discharge in-services when required.
• Complete all facility documentation forms and ensure MDS data is accurate.
• Attend rehab meetings and participate in facility committees as directed.
• Act as rehab delegate to the interdisciplinary team and assist in scheduling patients.
• Perform screens and coordinate billing for OT services rendered.
• Cover other therapists’ caseload during absences and supervise junior staff as needed.
• Maintain professional relationships with staff, patients, families and colleagues.
• Deliver in-service education on patient care or OT services as needed.
• Comply with facility policies, confidentiality and resident rights.
• Report on-time adherence to work hours and maintain a professional image.
• Meet productivity requirements and comply with HR policies.
• Contribute to a positive work atmosphere and assist team as directed.
• Perform other duties assigned by supervisor or Regional Manager.
• Adhere to all facility policies and procedures.
