Job summary

Warsaw Rehabilitation and Healthcare Center in Warsaw, NC is hiring an Occupational Therapist to provide comprehensive evaluations and develop treatment plans aligned with MD orders, goals, and patient needs.

You will document progress, coordinate with the interdisciplinary team, supervise junior staff, and ensure compliance with facility policies while delivering quality care to residents.

Qualifications

• Graduate of an accredited university with a B.S., MA, or M.S. or Doctorate in Occupational Therapy that is recognized by the AOTA.
• SNF experience preferred.
• Holds current license and/or registration in the state.

Responsibilities

• Provide a comprehensive occupational therapy evaluation based on MD orders.
• Document findings on the standardized evaluation form timely and accurately.
• Develop a comprehensive treatment plan with long and short-term goals, frequency, duration and modalities.
• Obtain MD orders for evaluations, treatments, recertifications and discharges.
• Deliver treatment using modalities and adjust as indicated while following precautions.
• Document daily participation and progress on progress notes.
• Update treatment plan monthly for recertifications and document discharge summaries.
• Prepare discharge summaries and in-service documentation as required.
• Complete any other documentation forms required by the facility.
• Document care plans and information needed for the MDS as deemed necessary.
• Attend facility and rehab meetings and participate in committees as indicated.
• Act as rehab delegate to the interdisciplinary team and schedule patients.
• Perform screens and bill patients accurately for occupational therapy services rendered.
• Cover other therapists' caseload during absences and supervise junior staff when needed.
