Job summary

Ramseur Rehabilitation And Healthcare Center in Ramseur, NC is seeking an Occupational Therapist to provide comprehensive evaluations and to develop individualized treatment plans for residents under MD orders.

You will document progress, collaborate with the interdisciplinary team, ensure timely billing and discharge planning, and supervise junior staff while upholding patient confidentiality and facility policies. Join a dedicated team focused on quality care and resident well-being.

Qualifications

• Graduate of an accredited university with a B.S., M.A., M.S., or Doctorate in OT recognized by AOTA.
• SNF experience preferred.
• Holds current license/registration in the state.

Responsibilities

• Provide a comprehensive occupational therapy evaluation based on MD orders.
• Document findings on the standardized evaluation form timely and accurately.
• Provide a comprehensive treatment plan including long and short term goals, frequency, duration and modalities.
• Ensure MD orders are obtained for evaluations, treatments, recertifications and discharges.
• Provide comprehensive treatment to patients utilizing modalities and modify treatment as indicated while adhering to precautions.
• Document patient’s daily participation and progress on progress notes timely.
• Document monthly updated treatment plan on recertification comprehensively and timely.
• Document patient’s discharge on a discharge summary as indicated.
• Attend facility and rehab meetings; participate in facility committees.
• Act as rehab delegate to the interdisciplinary team and supervise junior staff as needed.
