Job summary

Sweetwater Care in Selma, California is seeking Occupational Therapists to join an in-house therapy team dedicated to cognitive and ADL rehabilitation. The role focuses on evaluation, treatment planning, and collaborative care with the IDT. Licensure and NBCOT/AOTA compliance are required, with supervision of assistants available.

Qualifications

• Active State Licensure as an Occupational Therapist
• NBCOT certification or eligibility
• Experience evaluating and treating cognitive/ADL disorders is preferred

Responsibilities

• Screen and evaluate patients with cognitive and/or ADL disorders
• Develop and implement plans of care following regulatory standards
• Provide rehabilitative, skilled, and medically necessary treatment
• Document patient progress and outcomes in the chart
• Coordinate with rehabilitation team and family for discharge planning
• Supervise assistants and support staff as needed
• Participate in required meetings and trainings
