Qualifications

• Must be licensed and/or registered Occupational Therapist to apply.

Responsibilities

• Provide a comprehensive OT evaluation based on MD orders.
• Document findings on the evaluation form timely and accurately.
• Provide a comprehensive treatment plan with long- and short-term goals, frequency and duration.
• Ensure MD orders are obtained for evaluations, treatments, recertifications and discharges.
• Provide comprehensive treatment to patients using modalities and adjust as needed with proper precautions.

Education

OT degree (AOTA)

OTR NBCOT (state mandated)

OT license/registration in-state
