Qualifications

• Must be licensed and/or registered as an Occupational Therapist.
• Active Ohio OT license required or as applicable by state regulation.
• NBCOT-OTR designation recognized by state boards where required.

Responsibilities

• Provide comprehensive occupational therapy evaluations based on physician orders.
• Document findings on standardized forms in a timely and accurate manner.
• Develop and implement comprehensive treatment plans with goals, frequency, and modalities.
• Ensure orders for evaluations, treatments, recertifications and discharges are obtained.
• Deliver patient care while adhering to precautions and adjusting treatments as needed.
