Creekside Transitional Care & Rehab in Meridian, ID is seeking a full-time Occupational Therapist to join our in-house rehab program. The role involves screening and treating patients with cognitive and ADL impairments, developing care plans, and delivering evidence-based interventions.

Qualifications

• OT license is required.
• Open to Occupational Therapists at all experience levels. New grads welcome.
• Screen/evaluate patients with cognitive and/or ADL impairments and develop plans of care.
• Utilize standardized assessments and evidence-based interventions.

Responsibilities

• Screen and evaluate patients with cognitive and/or ADL impairments and develop care plans.
• Use standardized assessments and evidence-based practices.
• Provide rehabilitative treatment per regulatory requirements.
• Administer ADL retraining, therapeutic activities, exercises, and modalities.
• Assess progress and adjust treatments in the clinical record.
• Evaluate outcomes and refer as needed.
• Counsel patients and families and coordinate with care providers.
• Communicate progress with supervisor and team.
• Provide comprehensive discharge summaries.
