Job summary

Rochester Restorative Care Center is seeking an Occupational Therapist (OT) to assess and plan individualized therapy for residents, promoting independence and safety. You will work with an interdisciplinary team to implement therapeutic activities and adaptive equipment as part of a comprehensive care plan.

The role requires MN OT licensure, graduation from an accredited OT program, and strong knowledge of OT principles, with excellent communication and collaboration skills.

Qualifications

• Current and active OT license in Minnesota.
• Graduate of an accredited occupational therapy program.
• Strong knowledge of occupational therapy principles, techniques, and equipment.
• Excellent communication, organizational, and interpersonal skills.
• Ability to work collaboratively within a multidisciplinary team.
• Previous experience in long-term care or rehabilitation setting preferred.

Responsibilities

• Evaluate residents’ functional abilities and develop individualized treatment plans.
• Provide direct occupational therapy treatments, including therapeutic exercises, ADL training, and adaptive equipment recommendations.
• Document evaluations, progress notes, and discharge summaries in compliance with state and federal guidelines.
• Collaborate with physicians, nurses, and other therapy staff to ensure coordinated, resident-centered care.
• Educate residents, families, and staff on therapy interventions, adaptive techniques, and safe practices.
• Monitor resident progress and adjust treatment plans as necessary to achieve goals.
• Maintain professional and ethical standards, including adherence to Minnesota Board of Occupational Therapy Practice requirements.
