Job summary

Bremerton Trails Post Acute seeks an Occupational Therapist to lead and deliver high-quality therapy services in the facility. You will evaluate patients, develop treatment plans, and document progress while collaborating with an interdisciplinary team to achieve optimal function for residents.

Responsibilities include educating patients and families, supervising rehab staff, and ensuring compliant documentation and billing in the EMR system.

Qualifications

• Graduate of an approved Occupational Therapy curriculum.
• Active OT license in good standing.
• CPR certification at renewal.
• Experience in long-term care preferred.
• Strong English reading, writing, and verbal communication.
• Knowledge of Medicare and payor sources.

Responsibilities

• Provide evaluation, treatment planning, and progress documentation.
• Educate patients, families, and staff on therapy plans and outcomes.
• Supervise assistants and rehab techs; delegate tasks per guidelines.
• Enter patient data and billing information accurately in EMR.
• Collaborate with interdisciplinary team to optimize patient function.
