Job summary

SanStone Health & Rehabilitation seeks an Occupational Therapist to provide comprehensive therapy services in a skilled nursing facility. You will assess, plan, organize, and participate in rehab programs to improve mobility and independence in ADLs.

The role requires an OT degree and active licensure in North Carolina, with preference for 1–2 years in SNF settings; new grads are welcome. Full-time, on-site position with competitive benefits.

Qualifications

• Master's or Doctoral degree in Occupational Therapy from an accredited program.
• Active Occupational Therapy License in North Carolina.
• Experience in skilled nursing or long-term care preferred, new graduates welcome.

Responsibilities

• Evaluate residents' physical, cognitive, and emotional needs and develop individualized treatment plans.
• Implement evidence-based interventions to improve ADLs and mobility.
• Collaborate with a multidisciplinary team to coordinate care.
• Maintain accurate documentation of assessments, plans, progress, and discharge summaries.
• Educate residents and families on managing conditions and safety at home.
• Assist in discharge planning and home modifications recommendations.
