Progressive Rehab Solutions is seeking a licensed Occupational Therapist to evaluate, plan, and implement individualized therapy programs in long‑term care or related settings. You will lead patient assessments, develop treatment plans, supervise assistants, document outcomes, and collaborate with physicians, nursing staff, and families to promote independence and quality of life for diverse patients.

Must maintain licensure, stay compliant with regulations, and be able to travel to patient

Qualifications

• Master’s or Doctorate Degree in Occupational Therapy.
• Current state licensure as an Occupational Therapist.
• One year of experience preferred.

Responsabilités

• Evaluate patients’ physical, cognitive, and functional abilities using standardized assessments.
• Develop individualized treatment plans based on patient needs, physician orders, and regulatory requirements.
• Implement ADL/IADL interventions and train patients in adaptive equipment and compensatory techniques.
• Fabricate, fit, and train patients in orthotics and splints as appropriate.
• Educate patients and caregivers on therapy techniques, safety, and home programs.
• Complete clinical documentation accurately and timely in accordance with Medicare, regulatory, and company guidelines.
• Maintain records of patient evaluations, progress notes, and discharge summaries.
• Ensure compliance with federal, state, and payer regulations.
• Collaborate with interdisciplinary team and supervise COTAs and support staff as applicable.
• Engage in continuous professional development and evidence-based practice.
