Job summary

Sage Event Management Inc is seeking a full-time Occupational Therapist (OT) to join our in-house rehab team. You will work to improve residents’ independence, safety, and daily functioning through person-centered care and goal-oriented therapy.

Responsibilities include evaluating patients, developing individualized treatment plans, recommending adaptive equipment, collaborating with the care team, and documenting progress in Net Health EHR.

Qualifications

• Active Texas Occupational Therapy license required.
• SNF or long-term care experience preferred.
• Current knowledge of occupational therapy treatment practices.
• Experience with Net Health Electronic Documentation System preferred.
• Ability to manage patients with varying personalities and needs.
• Strong communication, collaboration, and clinical decision-making skills.

Responsibilities

• Consult with patients to evaluate their condition and impact on daily occupations.
• Assess and interpret patient evaluations and test results.
• Develop individualized treatment plans to maximize ADLs/IADLs.
• Assess home and living environments and recommend modifications.
• Provide training on adaptive equipment to support independence and safety.
• Deliver treatments per plan of care and monitor responses.
• Collaborate with interdisciplinary team to ensure high-quality care.
• Complete documentation timely and in compliance with policies and regulations.
• Attend required rehab department meetings.
