Qualifications

• Bachelor's or Master's degree in Occupational Therapy from an accredited institution.
• Current valid license to practice as an Occupational Therapist.
• Minimum 2 years of experience in a similar healthcare role.
• Experience with treatment planning, plan of care, and therapeutic activities.
• Experience in patient and caregiver education, safety and fall risk reduction.
• Familiarity with discharge planning and durable medical equipment.
• Strong collaboration with interdisciplinary healthcare teams.
• Confidentiality and HIPAA compliance awareness.

Responsibilities

• Developing comprehensive treatment plans tailored to each patient's needs and goals.
• Creating and implementing detailed plans of care, including therapeutic activities and functional task training.
• Providing education and support to patients and caregivers to enhance understanding and participation in the treatment process.
• Prioritizing safety awareness and fall risk reduction in all therapeutic activities.
• Conducting discharge planning and recommending appropriate durable medical equipment to support patient needs post-discharge.
• Collaborating with an interdisciplinary team of healthcare professionals to provide holistic care to patients.
• Ensuring all patient information is handled with utmost confidentiality, adhering to HIPAA compliance guidelines.
• Continually assessing and revising treatment plans as necessary, based on patient progress and feedback.
