Job summary

Horizon Home Care & Hospice, Inc. is seeking an Occupational Therapist to deliver in-home evaluation and therapy services. You will assess clients in their homes, plan interventions, and coordinate with care teams to ensure safety and positive outcomes.

The role covers Racine and Kenosha counties in Wisconsin, with no on-call, weekends, or holidays required, and a comprehensive benefits package to support work-life balance and professional growth.

Qualifications

• Graduated with OT curriculum accredited by CAM/AOTA.
• A current Wisconsin OT license is required.
• Home care experience preferred.

Responsibilities

• Provide evaluation and therapy services to clients in their homes.
• Assess physical, cognitive, behavioral, communication and safety needs.
• Submit discipline assessments, OASIS data sets and physician orders per policy.
• Cover Racine and Kenosha counties with travel as needed.

Responsible for providing evaluation and therapy services to clients in their homes.

• Assesses clients' physical, cognitive, behavioral, communication, emotional and social status; assesses home safety, medication management and fall risk.
• Assesses clients' functional rehabilitation status to determine the current "level of functioning, self-care, self-responsibility, independence and quality of life" per JCAHO Rehabilitation Standards.
• Submits accurate, complete and timely discipline assessments, OASIS data sets, physician orders and certifications / re-certification paperwork per Agency policy.

This position will cover Racine and Kensha Counties!

• Graduated after successfully completing an Occupational Therapy curriculum accredited jointly by the Council on Medical Education of the American Medical Association of the American Occupational Therapy Association
• A current license to practice in Wisconsin.
• Home care experience preferred.
