Job summary

Horizon Home Care and Hospice, Inc. is seeking a Physical Therapist to provide home-based evaluation and therapy in Racine and Kenosha Counties under physician direction.

You will work within an interdisciplinary team, delivering interventions to restore function and prevent disability and maintain patient quality of life. A CAPTE‑accredited degree and WI license are required, with at least 1 year of outpatient/home care experience, and a valid WI driver's license.

Qualifications

• Graduation from a CAPTE approved Physical Therapy Program.
• Current WI physical therapy license.
• 1 year of therapy experience (home care preferred).
• Current/valid WI driver's license; reliable/insured vehicle.

Responsibilities

• Assess clients’ physical, cognitive, behavioral, communication, emotional and social status.
• Assess functional rehabilitation status and independence per standards.
• Submit accurate OASIS data sets and physician orders per policy.
• Select and modify interventions based on problems, prognosis and goals.
• Establish a care plan with evidence-based interventions.
• Conduct regular case conferences and update plans as needed.

Job Overview

Horizon Home Care and Hospice is seeking experienced therapists to join our top‑notch Rehabilitation team. Our rehabilitation family consists of the very best physical therapists, speech‑language pathologists, occupational therapists, therapy assistants, and support staff. As a Physical Therapist, you will provide evaluation and therapy services in the homes of clients who require skilled intervention to restore function and prevent disability following disease, illness, injury, or loss of a body part within Racine and Kenosha Counties. You will take an interdisciplinary team approach, under the direction of a physician.

Responsibilities

• Assess clients’ physical, cognitive, behavioral, communication, emotional and social status; assess home safety, medication management and fall risk.
• Assess clients’ functional rehabilitation status to determine the current level of functioning, self‑care, self‑responsibility, independence and quality of life per JCAHO Rehabilitation Standards.
• Submit accurate, complete and timely discipline assessments, OASIS data sets, physician orders and certification/re‑certification paperwork per Agency policy.
• Select, apply and modify skilled interventions appropriate to the client’s environment based on identified problems, diagnoses, prognosis, anticipated goals, desired outcomes and client treatment choices.
• Establish a plan of care with interventions that reflect current, valid research findings supporting appropriateness to meet clients’ needs.
• Conduct case conferences with team members regularly, including creating PCW/HHA care plans, initiating and updating plans in accordance with patient needs and Agency policy.
