Job Overview

We are seeking an Occupational Therapist to provide evaluation and therapy services to clients in their homes in Racine and Kenosha counties.

Responsibilities

• Provide evaluation and therapy services to clients in their homes.
• Assess clients' physical, cognitive, behavioral, communication, emotional and social status; assess home safety, medication management and fall risk.
• Assess clients' functional rehabilitation status to determine the current "level of functioning, self-care, self-responsibility, independence and quality of life" per JCAHO Rehabilitation Standards.
• Submit accurate, complete and timely discipline assessments, OASIS data sets, physician orders and certification / recertification paperwork per agency policy.

Qualifications

• Graduated with a completed Occupational Therapy curriculum accredited by the Council on Medical Education of the American Medical Association and the American Occupational Therapy Association.
• Current license to practice in Wisconsin.
• Home‑care experience preferred.
• Current driver’s license for the State of Wisconsin and proof of current automobile insurance.

Schedule & Compensation

Part‑time: 20–32 hours per week. First shift: 8:00 am–4:30 pm. Remuneration: per visit, exempt.
