Job summary

Continuum Therapy Partners seeks a licensed Occupational Therapist to join our PRN daytime program in Mill Creek and Galion, OH. You will deliver care aligning with our standards, evaluate functional status, and promote independence for patients in diverse settings.

The role offers flexible PRN scheduling, a collaborative leadership team, and the opportunity to impact patient outcomes while maintaining clinical autonomy.

Qualifications

• Master's or Doctorate Degree in Occupational Therapy required.
• Current, unencumbered state Occupational Therapy License; NBCOT Certification required
• 1 year of previous experience as an Occupational Therapist preferred

Responsibilities

• Evaluates and develops individualized treatment plans to improve patients' functional independence
• Documents patient progress and modifies treatment plans based on clinical outcomes
• Educates patients and families on adaptive equipment and home safety recommendations
