Job summary

SSM Health in Monroe, WI, seeks a licensed Occupational Therapist to evaluate and treat patients with physical, cognitive and psychosocial deficits in a hospital setting. You will design and implement individualized treatment plans, educate patients and families, and collaborate with the care team.

The role requires state licensure in Occupational Therapy and BLS HCP within 90 days; no prior experience is required.

Qualifications

• Bachelor's, Master's or Doctorate in Occupational Therapy required to obtain licensure.
• OT licensure in state of practice required.
• BLS HCP certification within 90 days of employment may be required.

Responsibilities

• Assesses patient's ability to safely perform independent living skills and designs the plan of care.
• Implements treatment plan using appropriate modalities; documents progress and adjusts as needed.
• Educates patient and family about deficits and discharge planning; discusses adaptive equipment and support programs.
• Oversees care provided by designated individuals according to regulations and standards.
• Maintains alertness and safety in all activities.
• Performs other duties as assigned.
