Job summary

Aurora Health Care is seeking a licensed Occupational Therapist to join our care team in Wisconsin. You will evaluate patients, develop and implement individualized treatment plans, and coordinate discharge planning while ensuring safety and quality care.

Responsibilities include collaboration with physicians and staff, documentation, and adherence to accrediting standards. Clinical supervision may be required for assistants; BLS is required within 90 days.

Qualifications

• Master’s Degree in Occupational Therapy from an accredited school
• Licensed Occupational Therapist in the state where you work; BLS for Healthcare Providers per AHA
• No experience required

Responsibilities

• Interpret physician referrals and evaluate patient's physical, perceptual, cognitive, social, and vocational abilities.
• Establish a plan of care and implement treatment based on evaluation results and goals.
• Provide and/or fabricate assistive devices to enhance functional independence.
• Monitor outcomes and adjust treatment plans; communicate with physicians and teammates.
• Discharge planning with written home programs, family teaching, and referrals as needed.
• Document patient care, progress notes, and required department reports per policy.
• Maintain productivity standards and adhere to billing and documentation policies.
• May supervise therapy assistants and rehab aides per state regulations.
• Promote safety and participate in performance improvement activities.
