CareOne Home Health is seeking a Certified Occupational Therapist Assistant for our expanding home health services in Stanislaus and Merced Counties. Mileage reimbursement and paid driving time are provided.

Qualifications

• Graduate of an occupational therapy assistant program accredited by the American Occupational Therapy Association
• Certified by the National Certification Examination of the American Occupational Therapy Association
• Minimum of one year of clinical experience as a COTA
• Excellent verbal and written communication and organizational skills
• Current CPR certification
• Valid driver’s license with insured, well-maintained vehicle

Responsibilities

• Participate in evaluating patients' functional status and home environment hazards as delegated by the supervising registered occupational therapist
• Assist in teaching and retraining patients in daily activities affected by illness or injury
• Maintain accurate clinical records and report changes in patient condition
• Follow treatment plans and document patient responses and progress
• Coordinate with healthcare team members to ensure comprehensive care
• Attend in-services and team meetings
• Identify additional home care needs and assist with referrals
• Educate patients' families and caregivers on treatment regimes
• Engage in quality improvement activities
• Supervised by a registered occupational therapist at least every 30 days, with ongoing documentation of communication and patient outcomes
