Job summary

UVM Health Network - Home Health & Hospice seeks an Occupational Therapist to provide in-home OT services under moderate to minimal supervision.

Required: a Bachelor's degree in occupational therapy and at least one year of OT experience. You will assess patients, plan treatments, and coordinate care with families and clinicians to maximize independence at home. You will manage a patient caseload and document plans and progress.

Responsibilities

To provide a full range of occupational therapy services in a home care setting, under moderate to minimal clinical supervision

Education:

Experience:
* One or more years of general OT experience with Bachelor's degree

Performance Standards:

• Provide a full range of occupational therapy services to patients in their homes, under moderate to minimal personal and procedural clinical supervision, including: ongoing assessments of current status; implementation of treatment plans, including specific therapeutic exercises/activities and instruction in using adaptive techniques and equipment in order to regain maximum independence, and the like; teaching patients about their conditions and OT treatment plans, self-treatment and care, prevention of further injury, and the like; counseling patients regarding psycho-social problems related to physical disabilities; instruct and supervise LNA's in specific care with patients
• Admit patients and conduct comprehensive initial in-home assessments
• Manage a caseload of patients, including developing appropriate care plans and revising as circumstances change, scheduling visits, and so forth. Plan and coordinate a variety of services for patients provided by the University of Vermont Health Network Home Health & Hospice and other community health care and human services organizations.
• Confer with patients' families, physicians, and other care providers to coordinate services, discuss patients' status, needs and problems, and so forth
• Prepare all required clinical and administrative documentation related to assigned activities, including admissions write-ups, chart notes, documentation of conversations, care plans, and the like, adhering to all relevant legal regulations, technical requirements of government agencies, internal procedures, and professional standards
