Job summary

UVM Health Network - Home Health & Hospice seeks an Occupational Therapist to provide in-home OT services under moderate to minimal supervision.

Required: a Bachelor's degree in occupational therapy and at least one year of OT experience. You will assess patients, plan treatments, and coordinate care with families and clinicians to maximize independence at home. You will manage a patient caseload and document plans and progress.

Qualifications

• Bachelor's degree in occupational therapy or higher.
• One or more years of general OT experience.

Responsibilities

• Provide full range of OT services to patients in their homes under moderate to minimal supervision.
• Admit patients and conduct comprehensive initial in-home assessments.
• Manage a caseload of patients, develop/adjust care plans and schedule visits.
• Coordinate with patients' families, physicians, and other care providers to ensure appropriate services.
• Prepare all required clinical and administrative documentation related to assigned activities, including admissions write-ups and care plans.
