Job summary

Qualifications

• Admit patients and conduct comprehensive initial in-home assessments.
• Manage a caseload and develop appropriate care plans.
• Coordinate services with UVM Health Network Home Health & Hospice and community providers.
• Prepare clinical and administrative documentation in line with regulations and standards.

Responsibilities

• Provide full range of occupational therapy services in home care under moderate to minimal supervision.
• Admit patients and perform initial in-home assessments.
• Plan and coordinate various services and revise care plans as circumstances change.
• Confer with families, physicians and other care providers to coordinate services.
• Prepare all required documentation including admissions, chart notes and care plans.
