Northbrook Healthcare in Willits, CA, is seeking a Full Time Occupational Therapist to join our in-house therapy team. This position offers a $10K sign-on bonus for the next 30 days and a pay rate of $60–$70 per hour based on experience.

Qualifications

• Licensed Occupational Therapist with state licensure.
• Open to applicants of all experience levels.

Responsibilities

• Screen and evaluate patients with cognitive and/or ADL disorders and develop an appropriate care plan.
• Utilize standardized assessments and evidence-based practice to support clinical interventions.
• Provide rehabilitative, skilled and medically necessary treatment interventions to patients with cognitive and/or ADL disorders per regulatory and clinical practice requirements.
• Administer various procedures as part of the rehabilitation plan including ADL retraining, therapeutic activities, exercises, modalities and use of devices.
• Assess patient progress and adjust treatments in the clinical record per regulatory and clinical practice requirements.
• Evaluate treatment outcomes and refer as needed based on results.
• Provide consultation and counseling to patients, families, caregivers and other service providers.
• Communicate progress with supervisor and health team members regarding patient progress and plans.
• Provide comprehensive discharge summaries of services provided per regulatory requirements.

Key Responsibilities

• Effectively screens/evaluates patients with cognitive and/or ADL physical functioning disorders and develops appropriate plan of care following all regulatory and clinical practice standards.
• Utilizes standardized assessments and evidence-based practice to support clinical interventions.

• Assesses patient’s progress and adjust treatments accordingly in the clinical record per regulatory and clinical practice requirements.
• Evaluates treatment outcomes for effectiveness. Services are modified in relation to patient evaluation and referrals are made in relation to outcomes.
• Provides consultation and counseling to patients, as well as families, caregivers and other service providers related to the physical disorders.
• Effectively communicates with supervisor and other health team members regarding patient progress, barriers, and treatment plans.
• Provides comprehensive discharge summary of services provided per regulatory and clinical requirements.
