Trilogy Home Healthcare, a Humana company, is hiring a dedicated Occupational Therapist (OT) to join our award-winning team for home health visits in St Lucie and Indian River counties, FL. This full-time role offers a guaranteed salary and a collaborative culture with clinical autonomy.

You will deliver compassionate care, collaborate with doctors and interdisciplinary teams, help patients regain mobility after illness or surgery, educate families, assess home safety, and support patients

Qualifications

• OT license in Florida; active registration required.
• 1+ years of OT experience; home health experience preferred.
• Graduate of an accredited OT program (AOTA-approved).

Responsibilities

• Deliver compassionate, high-quality, personalized care in home health visits.
• Collaborate with doctors, Clinical Team Managers, and interdisciplinary teams.
• Help patients regain mobility, strength, and independence after illness or surgery.
• Educate patients and families on balance, walking, and preventing decline.
• Assess and ensure safe mobility in the home environment.

What You Bring:

• Graduate of an accredited school of Occupational Therapy Program (AOTA-approved)
• Active OT license in Florida
• Valid Florida drivers license, active vehicle insurance, and clear MVR
• 1+ years of experience as an OT required, Home Health experience preferred
• Strong initiative and independent judgement
